Complete and submit an online nomination form:

• Carer Representative
• Carer Organisation Representative

If you apply online, and your application is successful, we will ask you to provide proof of identity and a signed copy of the completed form.

By email or post

To apply by email or post you will need to:

1. Download and complete the Carer Representative or Carer organisation representative nomination form
2. Send us your completed application
3. Send us your curriculum vitae/resume outlining your relevant experience (no more than 2 pages).

You can email your completed nomination form to  Carers@dcssds.qld.gov.au or you can post it to:
