The Department of Families, Seniors, Disability Services and Child Safety is collecting your personal information for the purpose of assessing your application to become a member of the Queensland LGBTQIA+ Roundtable. As part of this process, your referees may be contacted, and your personal information, such as your legal and/or current name, background, qualifications/ experience and other information provided on this Application form, may also be disclosed to the following entities:
You may also be asked to consent to your personal information being disclosed to the Queensland Police Service for the purpose of conducting a criminal history check.
If your nomination is successful, some limited personal information will also be used for the administration of the Roundtable and its activities, including for the purposes of promotion of the Roundtable.
Your personal information will be handled in accordance with the Information Privacy Act 2009. Unless authorised or required by law, your personal information will not otherwise be disclosed to any other party without your consent.
